Fix/Complete Simple Review PHP/SQL Script

I'm in need of a skilled programmer who can enhance my current PHP script to embed it into my Wordpress theme. I have a basic understanding of php and sql databases but only enough to read and understand code not really create it. i’ve used chat GPT to create a very basic script and it’s not done a bad job however it’s unfinished. I can share my Chat GPT log so you can see the files created and understand the progress. It’s a simple script and most of the code is created. I need someone who can edit the code and fix the functionality. Here is what i was creating: The project is a web-based system for business reviews.
