Hi , this is simple design . Change r91 and r99 to 40k kohms and you will get gain 4. 40k resistor is not available in market, so we can use 39k +1k in series

I am not sure what you mean. Is it the job for layout or selection for parameters of the schematic? I can do simulation for OpAmp in LTspice and report the results. I think the gain is much more this. What is the voltage level input?
