Description: We need a functional mock up of a User adding or changing a Business Lisiting in our system. I have most of the pages in HTML so this work will be on script behind them

The system will have
1. An initial screen where users searches to see if a business exists.
2. User submits basic info for request - add or change
3. Request Are sent for Review
4. Admin Review and Accepts or Rejects or Holds Changes or Additions. If Reject or Hold - Notes are entered.

My budget is $50-$100 for this phase
